Social Prescribing - Community Approach To Health And Well-Being - What is Social Prescribing?

Social Prescribing is a programme aimed at helping people with mild mental/physical health issues. That could simply be isolation or mild depressions problems. It starts with  your social prescriber discussing with you a health plan best suited to answer your needs. At the heart of any health plan will be the use of locally based community projects.

Unfortunately our social prescribing project has run into financial difficulties, exasperated by the political stalemate. We are hoping to rejuvenate  it next year. However, we can still offer advice and support to anyone who would normally have availed of the service. 

A typical plan will entail.

  • Introduction meeting and assessment.
  • Discussion on local community based options.
  • Client agreement a bespoke plan of action with client.
  • Continued support from the social prescriber for an agreed time.
  • Follow up support if needed when bespoke plan has been completed.

This is a referral programme whereby medical practitioners can refer clients to the project. Self-referrals are not permitted but anyone who feels that they could benefit from this programme should discuss it with their doctor or other medical professional and ask them to make the referral.
